Job Description Job Description Case Manager Licensed Practical
Nurse (LPN). Responsibilities include- Maintain and document
clients medical necessity. Admit and Discharge clients. Complete
Admission documentation for clients enrollment. Manage clients
Med-list upon admission through discharge. Work with Clinical team
on clients treatment goals. Conduct education groups with proper
documentation as needed. Monitor and document the effectiveness of
treatment and medication. Maintain a courteous, and professional
appearance at all times. Follow facility policy and procedures as
outlined in the Employee Handbook. Follow a comprehensive plan of
care for individual clients. Coordinate with Home Health
facilities. Other duties may apply. Work hours Monday -Friday
8:00am - 5:00pm. Paid holidays and weekends off.
